Accessing the Patient Portal
First-time login must be made from a web browser.
- Select “Continue as a Patient”
- Username: Email Address
- Password: Create on Initial Login
Click in the upper left corner for your menu.
Home: request refills see upcoming appointments & recent messages
My Health: fill out your health history
Appointments: see your past and upcoming appointments and view your finalized visit summaries
Messages: send a message directly to your medical team, by listing “Nursing” in the “to” line of your
message
